EXTENDED CARE

Extended Care Services provides care in home for those patients that require continuous skilled nursing throughout the day and night as prescribed by their physician.

 

We can provide up to 24hrs of care, seven days a week based on medical necessity. Our focus is to ensure that the patient maintains a quality of life in the comfort of their family's home as opposed to being institutionalized in a nursing home or long term care facility.

Who is eligible?

- Medicaid recipients 0 through 20 years of age.

 

 

- Those patients that require multiple hours of skilled nursing.

 

 

 

- Recipients receiving extended nursing services must have a letter of medical necessity and a physician's prescription.
